Our Team Approach

Supporting men on their journey toward stability and independence requires a coordinated, multidisciplinary team. At SLPP, our staff provide compassionate, person-centered support 24 hours a day, 7 days a week, ensuring individuals have access to the programs, supports, and resources they need most. Together, we offer a safe, welcoming environment that promotes well-being, personal growth, and meaningful community connections.

Through a unified continuum of support, every team member plays a vital role in helping men to strengthen life skills, build resilience, foster meaningful connections, and move toward lasting stability and independence.

The Leadership Team guides SLPP’s strategic direction and ensures programs and services remain responsive, effective, and aligned with the needs of the individuals we serve. Their commitment to accountability, innovation, and continuous improvement strengthens core systems and supports a culture of excellence across the organization.

Management & Administration work as one integrated team responsible for the operational strength and reliability of SLPP. Management oversees day‑to‑day program delivery and consistency across all housing and support areas. Administration provides essential organizational infrastructure, including finance, human resources, IT, compliance, and systems management, ensuring SLPP operates smoothly and effectively.

Residential Support Workers (RSWs) are a vital part of SLPP’s day‑to‑day operations, providing consistent support grounded in compassion and empowerment. They foster positive rapport and create a welcoming environment where individuals feel respected, supported, and engaged in their goals. RSWs are often the first to welcome men to SLPP, offering practical assistance, guidance, and problem‑solving that helps individuals navigate daily challenges and strengthen the practical skills needed for stability and independence.

Care Coordinators deliver individualized, collaborative case management that supports men in identifying priorities, strengthening coping skills, developing life management strategies, and setting meaningful and actionable goals for long term stability. They provide coordinated access to essential community services and work with individuals to build individualized care plans that reflect their strengths, needs, and goals. Through this holistic approach, Care Coordinators empower individuals to build confidence, strengthen well being, and move toward greater independence.

The Intake & Reintegration Team is the first point of connection for individuals entering our housing programs. Their guidance and support lay the foundation for successful transitions, stronger community connections, and long‑term stability, helping individuals begin their journey with clarity and confidence.

The Health Team provides wellness supports, including health monitoring, medication administration, and individualized wellness planning. They collaborate with internal teams and external providers to help individuals manage wellness and improve overall health. Their role is essential in supporting individual’s wellness and ensuring individuals have access to the care they need.

Our Recreation Team delivers meaningful, structured programs that promote wellness, confidence, and social connection. Through recreation and leisure activities, the team reduces isolation, encourages engagement, and supports emotional and physical well‑being. Their programming helps individuals build motivation, strengthen community ties, and maintain a balanced lifestyle that compliments personal growth and long-term stability.

The Kitchen Team prepares nutritious, balanced meals every day, supporting physical health and overall well‑being. They accommodate dietary, lifestyle, and religious needs, provide packed lunches for individuals who work, and help create a sense of comfort, care, and routine within the community.

The Facilities Team maintains safe, clean, and functional spaces across SLPP. They ensure housing units, program areas, and common spaces are well‑maintained, respond to maintenance needs, and complete repairs that support safety, comfort, and reliability for individuals and staff.
